A true life story: how power failure killed my baby boy twin.

This is a true life story, I was mandated no to mention names, pictures and nor post videos.
A young girl who's newly married was taken to this hospital for delivery by her husband, she gave birth to a set of twins, a boy and a girl in a small new hospital or should I say clinic run by few staffs, a doctor, the metrons and some cleaners. The baby boy was overweight according to the doctor and the couple was adviced to put the baby on oxygen. The child was placed on oxygen in an intensive care unit ward.
You know PHED abi NEPA and their wahala, there was no light and theso farrefore the hospital was running on fueled generator.  The cleaner after cleaning, went to turn off the generator because she thought they had brought Nepa light.

People, that was how the little boy in the intensive support died. It has happened and blame was to be aportioned, the matron said she told the other cleaner who then told the other one who turned off the generator. The cleaner said she had forgotten that the baby was still in the intensive room and she thought the child was discharged.

Money was a compensation to the parents of the child, the doctor said they should pay half of the bill accrued and forget the rest. That was how the little boy's life was settled.

This incident happened in a respected city in the country.

My question is: WHO IS TO BLAME?
The epileptic power supply?
Employment of unqualified personnels?
Over stepping bounds in work place?
Lack of operations, cordination and office directives?
Lack of information at workplace?
The doctor?
The cleaner?
